Pupil Personal Accident Claims

Should you need to make a claim on this policy, following these few simple steps below will help to speed up the process considerably:-

The parent/guardian will complete a claim form (the claim form can be accessed below. Alternatively we can send it out to the school or parent/guardian on request).
This claim form will need to be signed by the parent/ guardian and the principal of the school. (If the policy is on an Option B basis the principal will have to check their records to see if the pupil in question is covered under the policy).
The form is then to be sent to Allianz with any accompanying medical/dental bills or receipts. If there are likely to be additional expenses as a result of the accident, please send a note with the claim form stating that and we will keep the claim open.
All correspondence should contain the policy number and/or reference number, together with the school phone number.

will cover pupils attending Vhi Swiftcare Clinics for treatment following an insured accident under our School Activities Cover or under our 24 hour cover.

Please forward all relevant receipts with a completed claim form to Allianz House, Elm Park, Merrionroad, Dublin 4.
